Preservação da fertilidade em pacientes oncológicas
O Instituto Nacional do Câncer (Inca) estima que 316.280 mil mulheres serão diagnosticadas com algum tipo de câncer em 2021. Deste total, cerca de 22% se referem a casos de pessoas com idades entre 20 e 49 anos, conforme indica um estudo feito em 2019 – “Câncer Antes Dos 50: Como Os Dados Podem Ajudar Nas Políticas De Prevenção?” -, que utilizou informações do Departamento de Informática do SUS (DATASUS) e do INCA, e traz como base os Registros de Câncer de Base Populacional (RCBP).
São crianças, adolescentes e jovens adultas que precisam ter acesso a informações sobre opções para preservação da fertilidade, que pode ser comprometida como consequência direta da doença ou efeito colateral de alguns dos tratamentos empregados. Alguns protocolos de quimioterapia e radioterapia podem acarretar na diminuição da reserva de óvulos ou até mesmo na perda da função ovariana, além de procedimentos cirúrgicos que necessitem remoção desses órgãos e/ou útero.
A oncologista Ana Carolina Branco, da Multihemo Oncoclínicas, explica que o tratamento oncológico inibe a proliferação de células cancerígenas, mas também pode interferir nas células germinativas. Por isso, a Sociedade Americana de Oncologia Clínica e a Sociedade Americana de Medicina Reprodutiva recomendam que as mulheres com diagnóstico de câncer devam ter acompanhamento de um especialista em preservação da fertilidade, sempre que possível, antes de iniciarem os seus tratamentos oncológicos.
“A linha de cuidado oncológico para mulheres jovens deve sempre levar em conta os aspectos relacionados à fertilidade para que a tomada de decisão sobre as terapias a serem empregadas considerem outros aspectos relativos à integralidade dessa pessoa para além da doença, tais como seus desejos futuros no tocante à maternidade. Por isso, é essencial a atuação conjunta de uma equipe multidisciplinar, que ajudará a paciente nesse processo de entendimento daquilo que pode ser feito no tocante à preservação das suas chances de gravidez futura”, diz Ana Carolina.
Ela comenta que, além do congelamento de óvulos, existem outras alternativas que podem ser eficazes, mas ressalta que é preciso avaliar caso a caso. “Essa técnica (congelamento) é indicada para mulheres no período reprodutivo e com boa reserva ovariana. Necessita de estimulação ovariana para coleta dos óvulos maduros que serão congelados. Esses óvulos, quando utilizados, serão para realizar a fertilização in vitro”, ressalta a médica da Multihemo Oncoclínicas.
Há ainda opções de congelar embriões; congelar o tecido ovariano (indicada para meninas que ainda não atingiram a puberdade, e mulheres que não podem atrasar o tratamento oncológico); bloqueio ovariano com medicação, que utiliza medicamentos para proteção ovariana durante a quimioterapia; e realocação dos ovários: técnica em que os ovários são deslocados da localização atual, através de cirurgia, para outra região do abdômen, diminuindo assim os riscos de serem acometidos.
